Form notes

Better questions make lighter work.

Practical notes on writing useful forms, keeping the respondent experience human, and turning a good answer into a clear next step.

Question design

The shortest form is not always the kindest form.

A useful question gives someone enough context to answer without guessing. Start with the decision you need to make, then work backwards to the smallest set of questions that supports it.

Intake

Collect context before the first conversation.

Good intake is not a wall of fields. Use a welcoming first step, make the path feel relevant, and ask only what your team can act on when the answer arrives.

Follow-up

A response is the beginning of the handoff.

Keep the respondent, their answer, and the next action together. A clear review status is often more useful than another notification.

A small checklist

Before you share a form

The first question explains why the form is worth completing.

Every required answer has a clear reason to exist.

The final screen tells someone what happens next.
